   That must have happened in 100,000 BC / The Unearthly Child http://en.wikipedia.org/wiki/Unearthly_child It must have been painted in the cave of skulls where the Doctor, Ian, Barbara and Susan were held captive. Susan was kind of an artist, maybe she did it.
